Position Summary
The Registered Nurse (RN) position will manage a panel of patients assigned within their territory by working directly with patients and collaborating with others involved. Every day, this colleague will focus on how to improve the care their patients receive. The position will be accountable for the alignment of direct patient care, management, and coordination in collaboration with others involved. This role will regularly communicate and meet with patients in the patients home, evaluate status, assess their records, monitor care delivery, document symptoms, and evaluate reactions and progress based on their plan. The colleague will assess, educate, and support the patient while reconciling changes in medications, managing medications, and care transitions. The role will be responsible for advocating for the patients care based on their unique needs, challenges, and preferences, while consulting with Spiras nurse practitioners and other healthcare providers. This role will also interact with patient families and care givers regularly as a core member of the Spiras Health Clinical Care Team. The role is foundational to the multi-disciplinary care team focused on delivering an exceptional patient experience that is well coordinated, clinically sound, and effective.

A typical day for one of our nurses may include:
Managing established patients via phone and at patient homes
Performing physical exam and diagnostic tests in the patients home
Identifying and managing patient barriers to care
Educating patient on self-care and disease management
Coordinating patient care with caregivers, family
Speaking with a patient on the phone about a prescription change
Meeting with a new patient to complete a full clinical assessment
Collecting patient health histories
Interpreting patient information and making recommendations to nurse practitioners
Consulting with nurse supervisors and physicians to determine best treatment plans
Coordinating care with other healthcare professionals, including licensed practical nurses, certified nurse assistants, nurse aides, etc. based on the unique situation
Administering medications, wound care, and other treatment options

Duties and Responsibilities
Monitor patients condition and assess needs to inform the treatment plan
Deliver education to the patient that aligns with their healthcare literacy level, motivations, desires, and preferences
Observe and interpret patients symptoms and communicate them to nurse practitioners
Collaborate with nurse practitioners to devise individualized care plans for patients
Perform routine procedures, vitals, qualitative assessments, etc.
Adjust patients medication and provide direction on treatments per nurse practitioner or physician orders
Adhere to hygiene and safety standards at all times (sanitizing surfaces, equipment, hands, etc.)
Document patient care accurately and timely per policy
Foster a supportive and compassionate environment to care for patients and their families
Other duties as assigned

Qualifications
5+ years of experience as a Registered Nurse
Diploma or Bachelor of Science degree in Nursing
Excellent knowledge of nursing care methods and procedures
Excellent knowledge of emergency care
Consistently adhere to documented policies, procedures, and standards
Valid and active state nursing license

Preferred Qualifications
7+ years of Registered Nurse experience
Experience with cardiac or pulmonology specialties
Experience with advance directives and palliative care
Experience with clinically complex geriatric patients
