Position Summary
Spiras Health is looking for a Licensed Practical Nurse (LPN) to join our team. This position is an essential component of our Spiras Care Team to fulfill care requirements of our patients who have chronic and complex diseases. Under the direction of the Nurse Practitioner and Registered Nurse, the LPN will be fully accountable for ensuring that patient care is delivered consistently, competently, and safely while providing routine care, observing patients health, monitoring health outcomes; and communicating instructions to patients regarding medication, home-based care, and preventative lifestyle changes. A typical day for the LPN may include:
Helping patients with ADLs
Monitoring patients overall health during each home visit
Changing bandages, wound dressings, and catheters
Setting up exercise programs for patients
Verifying patients are taking the correct dosages of medications
Remaining on-call for patients who have healthcare emergencies

Desired Characteristics
Strong and concise communicator able to rapidly identify and gather required information to assess and treat patients
Comfortable working with patients that may have difficulty hearing, struggle with memory issues, or have a lower health care literacy level
Willingness to collaborate with others
Ability to think quickly and provide information with confidence and clarity
Ability to manage competing priorities, organize work efficiently, and meet deadlines
Perseverance and self-management to achieve defined objectives

Duties and Responsibilities
To accomplish this job successfully, an individual must be able to perform each essential function satisfactorily.
Record a patients medical history accurately
Take and record measurements of blood pressure, temperature, heart rate etc.
Observe patients under treatment to identify progress, side-effects of medications etc.
Monitor patients condition including fluid intake and output and compose patient charts
Assist ailing patients in daily necessary activities such as eating, bathing etc.
Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
Collect samples for lab testing as assigned
Provide emotional and psychological support when needed
Communicate with patients family or friends to provide advice, comfort and release instructions
Accurate and timely documentation of patient care and services per policy

Qualifications
2+years of proven experience as LPN or similar role
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
Familiarity with home health, safety and sanitation standards and procedures
Understanding of confidentiality obligations and nursing best practices
Excellent knowledge of medical and hospital terminology
Good knowledge of MS Office and data entry
Good physical condition and stamina
Excellent communication and interpersonal skills
Compassionate and able to handle stress
Basic Life Support certification
Successful completion of LPN license program is required

Preferred Qualifications
Experience with cardiac or pulmonology specialties
Experience coordinating care with health plans and health systems
Experience with advance directives and palliative care
Home care experience a plus
