Care Coordinator

Job Description:

SFHC is accepting applications for our Care Coordinator position.

This full-time position is
responsible for providing care coordination and case management to SFHC patients and
individuals/families served. This will include working with medical providers, community
partners, local agencies, and state organizations. This position identifies, educates, supports, and
acts as an advocate for individuals and families by facilitating care coordination and referrals to
services that support clinical care including enrollment into insurance and benefit programs,
accessing transportation, housing food, and specialty care. This position is responsible for
conducting active outreach and assisting with the completion of application of health insurance,
community resources, and financial assistance programs.

Desired qualifications include a
Bachelor’s Degree in one of the following fields: social work, psychology, counseling, or nursing.
Any combination of education and work experience that would likely provide the required
knowledge and abilities is qualifying.

Required qualifications include at least 1 year experience in
a directly related field and the ability to work independently.
